I want to understand the proposed plan and participate in the process.

1. The concern each task addresses and how completion will be measured.
2. What my signature confirms on this specific document.
3. Which requirements are covered by a court order and which deadlines apply.
4. What services, referrals, and practical support will be provided.
5. How my questions, disagreements, or proposed corrections will be recorded.
6. Whether I can receive all pages and review them with my attorney before deciding.
7. Which review procedure and deadline apply to the issue I am raising.

Please provide the plan, relevant orders, and applicable appeal information.
